For nearly 30 years, Community Hospice of Northeast Florida has provided specialized end-of-life care for adults and children with all types of life-limiting illnesses, along with a wide variety of support services for family members. Our focus is on providing comfort, maintaining the dignity of the individual, and improving quality of life.

Through Community Hospice Foundation, Community Hospice of Northeast Florida raises the funds needed to provide comfort and support to patients and families at the end of life. Your contribution goes directly toward meeting the specific care needs of adults and children in Northeast Florida, along with professional education and community outreach programs that help ensure everyone in our community knows more about end-of-life issues and has access to our services.

Gifts make a difference in the quality of end-of-life care in our community, specifically making an impact in the following areas:

• Patient care that is unfunded or under-funded by insurance reimbursement
• Emergency assistance for patients and patients' families with financial need
• Equipment for patient care
• Facilities and endowment
• Under-funded programs of Community Hospice, including Community PedsCare, our pediatric hospice and palliative care program for children with life-limiting or life-threatening illnesses.
